Their Place In Home Supports Program

The In Home Support Program at Their Place is a person centered-driven, outcome-based service tailored to the participant’s needs, wants, and preferences through the implementation of valued outcomes and goals.

We work with program participants, their families or a chosen network of support to create and monitor comprehensive habilitative goals. We empower everyone to obtain their goals in order to increase their independence and ability to be active participants in their community. Their Place provides one on one assistance to each participant, whether they live in a family setting or independently.

The In-Home Supports services include adaptive skill development, training and support for independence in living and travel adult education, development of social and informed choice skills, self-advocacy, and guidance accessing one’s community.
